During the safety inspection, we discovered the operator bracket was barely secured to the door stile—two of three screws were nearly backed out completely. We also found the door was extremely heavy when disengaged from the opener. After weighing the door, we determined the current springs were undersized, causing the opener to strain when lifting. We recommended replacing the failed operator bracket with an adjustable operator bracket and replacing the springs with properly sized units.
